Prayer for a sick man

מִי שֶׁבֵּרַךְ אֲבוֹתֵינוּ אַבְרָהָם יִצְחָק וְיַעֲקֹב, מֹשֶׁה וְאַהֲרֹן, דָּוִד וּשְׁלֹמֹה, הוּא יְרַפֵּא אֶת בֶּן בַּעֲבוּר שֶׁ בֶּן יִתֵּן בְּלִי נֶדֶר לִצְדָקָה בַּעֲבוּרוֹ, בִּשְֹכַר זֶה הַקָּדוֹשׁ בָּרוּךְ הוּא יִמָּלֵּא רַחֲמִים עָלָיו לְהַחֲלִימוֹ וּלְרַפְֹּאתוֹ וּלְהַחֲזִיקוֹ וּלְהַחֲיוֹתוֹ, וְיִשְׁלַח לוֹ מְהֵרָה רְפוּאָה שְׁלֵמָה מִן הַשָּׁמַיִם לְרַמַ"ח אֵבָרָיו וּשְׁסָ"ה גִּידָיו בְּתוֹךְ שְׁאָר חוֹלֵי יִשְׂרָאֵל, רְפוּאַת הַנֶּפֶשׁ וּרְפוּאַת הַגּוּף, וְנֹאמַר אָמֵן:

May He who blessed our fathers, Abraham, Isaac and Jacob, Moses and Aaron, David and Solomon, bless the sick person (name) the son of (mother s name) because (Say the donor s name) pledged charity, without a vow, for his sake. It is Shabbat say, Amen.

Prayer for a sick woman

מִי שֶׁבֵּרַךְ אֲבוֹתֵינוּ אַבְרָהָם יִצְחָק וְיַעֲקֹב, מֹשֶׁה וְאַהֲרֹן, דָּוִד וּשְׁלֹמֹה, הוּא יְרַפֵּא אֶת בַּת בַּעֲבוּר שֶׁ בֶּן יִתֵּן בְּלִי נֶדֶר לִצְדָקָה בַּעֲבוּרוֹ, בִּשְֹכַר זֶה הַקָּדוֹשׁ בָּרוּךְ הוּא יִמָּלֵּא רַחֲמִים עָלֶיהָ לְהַחֲלִימָהּ וּלְרַפְֹּאתָהּ וּלְהַחֲזִיקָהּ וּלְהַחֲיוֹתָהּ, וְיִשְׁלַח לָהּ מְהֵרָה רְפוּאָה שְׁלֵמָה מִן הַשָּׁמַיִם בְּכָל אֵבָרֶיהָ וְגִּידֶיהָ בְּתוֹךְ שְׁאָר חוֹלֵי יִשְׂרָאֵל, רְפוּאַת הַנֶּפֶשׁ וּרְפוּאַת הַגּוּף, וְנֹאמַר אָמֵן:

Upon completion of the final aliyah the Torah is covered and the Hatzi-Kaddish is said.

יִתְגַּדַּל וְיִתְקַדַּשׁ שְׁמֵהּ רַבָּא. בְּעָלְמָא דִּי בְרָא כִרְעוּתֵהּ וְיַמְלִיךְ מַלְכוּתֵהּ, וְיַצְמַח פֻּרְקָנֵהּ וִיקָרֵב מְשִׁיחֵהּ. בְּחַיֵּיכוֹן וּבְיוֹמֵיכוֹן וּבְחַיֵּי דְכָל בֵּית יִשְׂרָאֵל, בַּעֲגָלָא וּבִזְמַן קָרִיב. וְאִמְרוּ אָמֵן:

Exalted and hallowed be His great Name (Cong: Amen.) throughout the world which He has created according to His will. May He establish His kingship, bring forth His redemption and hasten the coming of His Mashiach (Cong: Amen.) in your lifetime and in your days and in the lifetime of the entire House of Israel, speedily and soon, and say, Amen.

(Cong: אָמֵן. יְהֵא שְׁמֵהּ רַבָּא מְבָרַךְ לְעָלַם וּלְעָלְמֵי עָלְמַיָּא, יִתְבָּרֵךְ:)

(Cong: Amen. May His great Name be blessed forever and to all eternity. Blessed.)

יְהֵא שְׁמֵהּ רַבָּא מְבָרַךְ לְעָלַם וּלְעָלְמֵי עָלְמַיָּא, יִתְבָּרֵךְ, וְיִשְׁתַּבַּח, וְיִתְפָּאֵר, וְיִתְרוֹמֵם, וְיִתְנַשֵּׂא, וְיִתְהַדָּר, וְיִתְעַלֶּה, וְיִתְהַלָּל, שְׁמֵהּ דְקֻדְשָׁא בְּרִיךְ הוּא. לְעֵלָּא מִן כָּל בִּרְכָתָא וְשִׁירָתָא, תֻּשְׁבְּחָתָא וְנֶחֱמָתָא, דַּאֲמִירָן בְּעָלְמָא, וְאִמְרוּ אָמֵן:

May His great Name be blessed forever and to all eternity. Blessed and praised, glorified, exalted and extolled, honored, adored and lauded be the Name of the Holy One, blessed be He, (Cong: Amen.) beyond all the blessings, hymns, praises and consolations that are uttered in the world; and say, Amen. (Cong: Amen.)

Before raising the Sefer Torah it should be rolled open to reveal at least three columns and one of the seams. When it is raised it should be turned to left and to the right so that the text is seem by all. As it is raised the congregation should stand, look at the Sefer Torah and say:

וְזֹאת הַתּוֹרָה,

This is the Torah

אֲשֶׁר־שָׂם משֶׁה לִפְנֵי בְּנֵי יִשְׂרָאֵל:

which Moses placed before the children of Israel.

עֵץ חַיִּים הִיא לַמַּחֲזִיקִים בָּהּ, וְתֹמְכֶיהָ מְאֻשָּׁר.85 דְּרָכֶיהָ דַרְכֵי נֹעַם, וְכָל נְתִיבוֹתֶיהָ שָׁלוֹם.86 אֹרֶךְ יָמִים בִּימִינָהּ, בִּשְׂמֹאלָהּ עֹשֶׁר וְכָבוֹד.87 יְיָ חָפֵץ לְמַעַן צִדְקוֹ, יַגְדִּיל תּוֹרָה וְיַאְדִּיר:

It is a tree of life for those who hold fast to it, and those who support it are fortunate. Its ways are pleasant ways, and all its paths are peace. Long life is at its right, riches and honor at its left. The Lord desired, for the sake of his [Israel s] righteousness, to make the Torah great and glorious.
